Job Description:
As one of the nation’s largest hospital based home care agencies, we are responsible for over 700,000 visits annually serving all five boroughs as well as Nassau/Suffolk. Our programs are licensed by the New York State Department of Health and accredited by JCAHO. We provide skilled nursing, rehabilitative and social work services to New Yorkers in every stage of life. We offer home based infusion therapy, skin and wound care and home care for heart disease, asthma, orthopedics and neo plastic problems. An interdisciplinary team works together to provide the highest level of care to individuals who choose to remain at home.
Plans, develops, implements and evaluates long term care services in accordance with the Agency’s vision to improve organizational performance in the delivery of quality care. Provides oversight of all services provided by the LTHHCP program.
NYS RN License is required. Bachelor’s degree is required. Master’s preferred. Three to five years satisfactory experiences in a Certified Home Health Agency or Long Term Home Health Care Program. LTHHCP experience is required and home care experience should include direct care and a minimun of 2 to 3 years in supervisory/management position.
